On the date of your scheduled appointment, please ensure to bring the following:
1. Your completed patient registration packet – Download & Print.
2. Your government issued identification card (ID).
3. Your insurance card.
4. All medications or a list of medications you are presently taking.

- We Can Help! You may be eligible for a discounted cost on our services, EVEN IF YOU HAVE INSURANCE! The sliding fee discount applies to all our services.
- What is the Sliding Fee Discount? A program designed to reduce the cost of primary care for those who meet income requirements. These low fees are available to patients based on the guidelines provided annually by the federal government.
- How Do I Qualify? You qualify for the program if you meet the eligibility guidelines. Sliding fee discounts are determined according to household size and yearly income, regardless of health insurance status.
- We promise to understand. We recognize that not all fee-for-service balances are due to a patient's unwillingness to pay, as there is often an inability. This practice serves all patients regardless of ability to pay.
- We comply with Federal civil rights laws and do not discriminate based on race, color, national origin, age, or disability, or genetics, and, where applicable, sex, marital status, parental status, religious creed, political beliefs, veteran status, and sexual orientation.
What We Ask of You
We make appointments for office visits. If you cannot keep your appointment, please call and tell us the day before so we can give that time slot to someone else.
If you need to talk with the physician about more than one problem, please tell the receptionist when you make your appointment so she can give you more time with the medical team.

Here at Singleton Health Center/Medical Center of Santee we are committed to providing you with the highest quality and most compassionate care, in a family atmosphere. We are part of your health care team so if you call with medical questions or needing refills ask for one of the nurses.
Refills
Many medications require ongoing refill, however, it is possible to run out of your medications prior to your next appointment. Please call during our normal business hours for a refill when you have at least 5 days of medication remaining. Also, please remember to bring all medications to every doctor’s visit.
Referrals
You may be referred to a specialist for additional care. Please allow the referral coordinator at least 7 business days to secure this appointment for you. Keep in mind we are doing all we can to provide you with the best continued care. However, it is the specialty office that schedules your appointment then informs us.
Patient Portal
Through the Patient Portal, you can communicate with your doctor and access important information through a secure internet connection. You now have 24/7 access to your medical information from the comfort and privacy of your own home or office.

The National Committee for Quality Assurance (NCQA) defines the Patient Centered Medical Home as “a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner”.
